The Service Provider agrees to accept fluvaccinationvouchers.com.au Flu Vaccination Vouchers as full payment for the services provided to persons presenting valid flu vaccination vouchers.
Fluvaccinationvouchers.com.au acknowledges that the service provider will not necessarily be able to deliver services at all times and to all persons presenting for vaccination.
Service providers warrant that they will:
Provide services in accordance with the requirements of relevant State/Territory Health Legislation and Regulations;
Ensure their staff are appropriately qualified, credentialed and experienced to deliver the Services; and
Maintain appropriate professional indemnity and public liability insurance for all risks associated with the delivery of the services.
If circumstances change and the service provider can no longer warrant the above requirements, the service provider must notify fluvaccinationvouchers.com.au via email (firstname.lastname@example.org) within 14 days at which time they will be removed from the program until notification is received that the above requirements are being met.
Flu Vaccination Vouchers may only be accepted by the Service Provider in exchange for flu vaccination services. Vouchers must not be exchanged for cash.
The Service Provider must not charge the person presenting the voucher a handling fee for accepting the voucher.
Prior to the provision of services, the service provider is responsible for confirming that the voucher is valid and has not previously been redeemed using the ‘Check Voucher’ portal and fluvaccinationvouchers.com.au will not be responsible for costs incurred where services have been provided in exchange for an invalid or redeemed flu vaccination voucher.
To redeem a voucher, the Service Provider must use the ‘Redeem Voucher’ portal at the time services are provided and provide the Service Provider’s bank account details. This will generate and send a tax invoice to fluvaccinationvouchers.com.au on behalf of the Service Provider.
Fluvaccinationvouchers.com.au agrees to pay the Service Provider by bank transfer directly into their nominated Bank Account within seven working days for vouchers recorded on the fluvaccinationvouchers.com.au portal as redeemed by the Service Provider.
Neither party will be liable for any breach of these terms and conditions where caused by circumstance beyond their reasonable control.
The fluvaccinationvouchers.com.au website (www.fluvaccinationvouchers.com.au) is a trading name of VaxWorks Health Services Pty Ltd (ABN 41 164 377 753).
The laws of Queensland shall govern this agreement.
